Overview

PASSPORT is an Ohio Medicaid program that helps eligible older adults receive care and support in their homes instead of in nursing homes. It is Ohio’s in-home alternative to nursing home care. Council on Aging (COA)  administers the PASSPORT Program in Butler, Clermont, Clinton, Hamilton, and Warren counties.

The PASSPORT program, which provides health and social care services to eligible individuals, is currently experiencing significant delays in service delivery due to its complex and inefficient processes. The program needs to be simplified at multiple touchpoints, including the process of engaging providers, referrals, follow-ups, and coordination with medical professionals.
